Sliding Fee Discount Program
The Sliding Fee Discount Program is based on household income relative to the Federal Poverty Guidelines. Refer to the table below to determine the discount for which you qualify.
Charity Care Discount Program
The Charity Care Discount Program is considered for extraordinary circumstances at the sole discretion of the COCPW Financial Assistance Committee. These circumstances may include, but are not limited to recent loss of employment, death in the family, physical disability, mental illness, and/or financial hardship.

Who is eligible for financial assistance at COCPW?
Financial assistance is available to all patients, with or without insurance, based on household income, family size, and extraordinary circumstances such as job loss or medical hardship.
What types of fees are covered under the financial assistance program?
Eligible fees include non-covered clinic services like co-pays, co-insurance, deductibles, and patient balances not covered by insurance. Certain supplies for self-pay patients may also be eligible.
Are there any fees that are not eligible for financial assistance?
Yes, services or supplies purchased outside COCPW (like labs, imaging, or drugs) and fees from no-shows, late payments, or overdue balances are not covered.
How do I apply for financial assistance?
You can complete an application available in English and Spanish at any COCPW clinic or on our website. Our staff is happy to assist you with the process.
How often do I need to reapply for financial assistance?
You must complete a new application every 12 months or anytime your financial situation changes.
Can financial assistance be applied retroactively?
Yes, approved discounts can be applied to eligible balances up to 3 months prior to your application date, provided you submit the required supporting documents.
Will my financial assistance affect my insurance claims?
No, but financial assistance depends on your continued cooperation with COCPW’s efforts to secure fair reimbursement from your insurer.
